Please submit the below sponsorship agreement form after it is processed you will receive an invoice that can be paid online with credit card. Please note: Your sponsorship is not secured until payment is made. If you are a current TAHP member in good standing, your member 10% discount will be applied at the time of invoicing.

Once your payment has been processed you will receive an email with instructions on registering your included attendees and additional event information. If you have questions please contact Patti Doner at pdoner@tahp.org.
