Please submit the below sponsorship agreement form after it is processed you will receive an invoice that can be paid online with credit card. If you are a current TAHP member in good standing, your member 10% discount will be applied at the time of invoicing.

Please note: Your sponsorship is not secured until payment is made and your sponsor page will not be activated until your invoice is paid in full.

To receive sponsor correspondences:
